Toll Fraud in VoIP: Attack Vectors, Real-Time Detection, and Response Playbook

Toll fraud against VoIP infrastructure is one of the fastest and most financially damaging attack types in telecommunications security. Unlike data breaches where damage may accumulate slowly over weeks, a successful VoIP toll fraud attack can generate tens of thousands of dollars in PSTN termination charges within hours—charges that the victim organization is contractually liable for under most SIP carrier agreements, regardless of whether the organization authorized the calls.

Common VoIP Toll Fraud Attack Vectors

    • PBX credential compromise: Attackers scan for SIP registrations using weak or default credentials. Once a SIP extension is compromised, calls are placed through your PBX account to high-cost international destinations—premium rate numbers in Eastern Europe, West Africa, and Pacific island nations that pay commissions to the number holders. This is the most common attack type against call centers.
    • SIP port scanning and brute force: Attackers scan the internet for open SIP ports (5060/UDP, 5061/TCP) and attempt to register as extensions using wordlist attacks. Any SIP service exposed directly to the internet without IP allowlisting or fail2ban-equivalent protection will be attacked within minutes of deployment.
    • Dialplan exploitation: Misconfigured IVR or auto-attendant systems that allow callers to dial external numbers (e.g., transferring to "9" + number) can be exploited to place unauthorized outbound calls through your PSTN trunk.
    • Account takeover via API: Cloud PBX and dialer platforms with exposed APIs and weak authentication can be exploited to configure new extensions, add outbound routes, or disable spend limits.

    Detection

"The average toll fraud attack runs for 4–6 hours before detection. Real-time spend alerts cut losses by 90%."

Effective toll fraud detection requires real-time monitoring of call volume, destinations, and per-account spend velocity—not just end-of-day CDR review. Set hard spend limits at the SIP carrier level that automatically suspend outbound calling when the limit is reached. Configure alerts for any outbound calls to international destinations your business does not legitimately call. Monitor for extension registration from IP addresses outside your known agent and office ranges.

Immediate Response Playbook for Active Fraud

    • Immediately disable outbound calling at the SIP carrier level—not just at the PBX level. If the PBX is compromised, PBX-level restrictions may not be honored.
    • Force a password reset on all SIP extension credentials and API keys. Identify the compromised credentials by reviewing SIP registration logs for registrations from unfamiliar IP addresses.
    • Contact your SIP carrier's fraud team immediately. Many carriers will waive or reduce fraudulent charges if reported within 24 hours, especially for first-incident customers. Document everything—timestamps, call logs, and evidence of unauthorized access—for the carrier dispute process.
    • After remediation, implement IP allowlisting for SIP registrations, deploy fail2ban or equivalent brute-force protection on all SIP-exposed ports, and enable SRTP for media encryption to eliminate media interception as a secondary attack vector.
